[#135] Provide Proper Licensing Information

This PR provides appropriate GPL3 boilerplate headers to the source code as well as a LICENSE file. While copyright on the legacy code is more than shaky (because neither copyright nor licensing has been managed properly) we hope that this provides a clean cut and a clean way forward.
